Individual and Community Based Supports

Precious Sapphires Healthcare, Inc. provides community-based supports through direct support with our skilled and trained staff for individuals, with or without their caregiver participating. Our services may be within the residence of the individual as well as throughout the community. Our supports are used to promote independence and choice to achieve or maintain the goals for independence, efficiency, promotion of family integration, and inclusion in the community based upon the Individual’s Service Plan. Community-Based Supports are delivered one-on-one with an individual and may include but are not limited to:

Assistance with community-based activities with training and supervision for activities such as:

Medical or Psychiatric Appointments
Day Program
Shopping
Community Activities of the individual’s choices planned that week
Cultural Events
Sporting Events
Skill building activities such as cards games, board games, gaming tournaments
Civic activities that promote inclusion and contribution to the communities in which individuals reside
Summer enrichment activities that includes daily and weekly camps

Transportation is provided for all activities by Precious Sapphires Healthcare Inc.

Eligibility

Precious Sapphires Healthcare, Inc. is proud to partner with the NJ Department of Developmental Disabilities (DDD) to offer Transportation Services, Supportive Employment, Individual Supports, Community Inclusion, and Community Based Support to individuals with intellectual and developmental disabilities that are eligible for waiver services.
